LEICESTER CITY - April 2008
The Brief
To redesign
the Intermediate Care Service to
address local healthcare challenges and bring change to the local
system.
The Challenge
Like many
cities, Leicester City health community
had faced significant challenges in meeting the complex health care
demands and levels of the local population. The PCT had put in place
new financial parameters to meet its responsibilities and the Leicester
University College Hospital had serious challenges in relation to
levels of admissions, delayed discharge and length of bed stays.
The Solution
We deployed
our Goal
Seeking Commissioning Model
which combines detailed service analysis
of activity, quality and outcomes with statistical analysis and
business modelling. The philosophy behind this model is co-production
and co-design with frontline staff, clinicians and managers to build
consensus towards delivering effective change.
Outcome
Leicester
City Health and Social Care partners
got a detailed blueprint for action to meet their targets based on
sound qualitative and quantitive modelling and consensus across the
range of services involved.
